Aultman OneChart Video Tutorials
Need help logging in or using the Aultman OneChart patient portal? We have a series of instructional videos for you! Click the links below to see quick, “how-to” videos with step-by-step instructions and visuals to help you find what you need!

You must be 18 years or older to use self-enrollment. Self-enrollment is for patient use only. You will need your medical record number to enroll. You can find this number on your discharge paperwork. Please contact Medical Records at 330-363-6256 if you need to obtain your medical record number. You can enroll by clicking here.
Your invitation will be sent at the time of registration. You will receive a reminder email if you have not set up your account after seven days. The invitation will expire after 90 days. Please email patientportal@aultman.com if your invitation has expired and a new invitation will be sent. This email address is monitored 8 a.m. through 4:30 p.m. Monday through Friday. Please allow for a 24-hour turnaround time during business hours.
Yes, you can access the portal via your smartphone or tablet by going to aultman.org/AultmanOneChart.

Go to http://myaultman.iqhealth.com/ and click on the “Forgot Password?” to reset your password.
Step-by-Step Instructions:
- Navigate to the Login Page: http://myaultman.iqhealth.com/
- Select "Forgot Password": Locate and click Forgot Password?
- Enter Your Email: Type in the email address you originally used to register for your patient portal account.
- Check Your Email: Open your email inbox and find the message from the portal. (If you don't see it, double-check your spam or junk folder).
- Create a New Password: Click the reset link or button in the email. You will be redirected to the portal to create a new password. Your password must typically be at least 8 characters long and include a mix of uppercase/lowercase letters, a number and a symbol.
If you have forgotten or no longer have access to the email address you originally used to register for your patient portal account, please email us at PatientPortal@Aultman.com. You can request that we change the email address associated to your current email address. We will then send you a secure email to verify your identity.
Please note that it takes 24 hours for the current account to be disassociated from the current email address. After we have verified your identity, you will receive the invitation at your new email address within 2 – 3 business days.
You can access your information by going to aultman.org/AultmanOneChart and entering your log in credentials.
Test results may show up in various sections on the Aultman OneChart patient portal. If you are looking for a recent result and do not see it on the portal home page, please click on “Health Record” at the top of the home page. From there, you can review results in many different sections, including Radiology, Pathology and Microbiology.
Please use the “Learn More” link at the top of the ‘Medication and Results’ section. If you have further questions after visiting these references, please ask your physician during your next visit.
